Supplemental Newborn Screening
Information for Physicians and Their Staff

Thank you for your interest in the Supplemental Newborn Screening Program available through the Institute of Metabolic Disease (IMD) at Baylor Research Institute.
Upon your request, the following information packet will be sent to assist you in evaluating our Supplemental Newborn Screening program:

Two Types of Contracting Options
There are two different types of contracting options available to your practice:
- One contract indicates that your patients will provide payment at the time of specimen submission to our laboratory; this contract is labeled in the footer as "Physician - Direct Pay".
- One contract provides for billing of your practice; this contract is labeled in the footer as "Physician - Billed".
Once you have selected the most appropriate contracting situation for your practice, please make a copy of that contract, sign both copies and forward both copies to our office. Upon receipt, we will sign both copies and return one to your office for your records. This will allow both parties to have a copy with original signatures.

Once the Signed Contract is received
Once a signed contract is received, the Institute will forward the following materials to your office:
- Informational brochures explaining Supplemental Newborn Screening to your patients
- A brochure holder
- Information on specimen collection for office staff
- Screening cards
- Glassine storage envelopes with informed consent attached; the glassine envelopes are used for storing cards once specimen is collected.
- Samples of the items described above
Please note that a physician's signature and a signed informed consent are required for each Supplemental Newborn Screening specimen submitted. The signature of an individual designated by the physician to sign on his/her behalf will be accepted. A document indicating this approval must be on file at the physician's office and a copy of the document must be available to Baylor upon request. Screening or resulting of a specimen may be delayed if the card or informed consent form is incomplete.
All screening results will be sent to the ordering physician and/or person(s) designated in your contract.
Supplemental newborn screening is not appropriate for the symptomatic patient or for patients with a family history of inherited metabolic disease. Advanced diagnostic evaluation and testing is available upon request. STAT analyses will not be performed on Supplemental Newborn Screening samples.
