Contract Information
Thank you for your interest in the Supplemental Newborn Screening Program available through the Institute of Metabolic Disease (IMD) at Baylor Research Institute.
Upon your request, the following information packet will be sent to assist you in evaluating our Supplemental Newborn Screening program:
• General Information regarding the screening program
• Screening card information and Specimen Collection Instructions sheet
• Articles regarding Supplemental Newborn Screening
• A sample screening card
• A glassine storage envelope with informed consent attached
• Patient Educational Brochure
• Examples of Result Formats
• 2 copies of the Newborn Screening Services Agreement Contract
• Client Information sheet
• Current copies of Institute's CAP and CLIA licenses
Once a signed contract is received, the IMD will provide the following materials to your facility free of charge:
• Screening cards
• A glassine storage envelopes with informed consent attached
• Patient Educational Poster
• Patient Educational Brochures and a brochure holder
All screening results will be sent to the person(s) designated in your contract. It will be the responsibility of this person(s) to relay the results to the ordering physician. The ordering physician should then share the results with the parent or guardian. Consultation will be provided when an abnormal screening result is obtained. Suggestions and recommendations for further diagnostic testing will be provided when appropriate.
Supplemental newborn screening is not appropriate for the symptomatic patient or for patients with a family history of inherited metabolic disease. Advanced diagnostic evaluation and testing is available upon request. STAT analyses will not be performed on Supplemental Newborn Screening samples.
