Complete the Application Form and the Externships Policy Form. Submit these two completed documents, a current photograph, and all documents listed on the application form to the Physical Medicine and Rehabilitation program address provided.

The application and supporting documents will be reviewed by both the PM&R Residency Program Director and the Director of Medical Education at Baylor. When the reviews are completed, the student will be notified either by letter or by e-mail regarding the decision on his/her application. For approved electives, a student's first choice of rotation periods is selected, to the extent possible.
