Baylor Health Care System: Graduate Medical Education Employee Benefits and Pre-employment Requirements


	As an employee at Baylor, you’re entitled to a host of benefits.

Before you begin to work for Baylor, we need written documentation of the following immunizations:
	Physician proof of diagnosed measles, two doses of live measles vaccine on or after first birthday, or laboratory evidence of measles immunity
	Physician proof of diagnosed mumps, laboratory evidence of mumps immunity, or proof of vaccine on or after first birthday
	Laboratory evidence of rubella immunity or proof of live rubella vaccination on or after first birthday
	Hepatitis B vaccine series of three
	PPD tuberculin test less than six months old, documentation of previous positive PPD, or history of prophylactic treatment for previously positive PPD
	Chickenpox - Titers or statement
