• Curriculum Vitae
• Official transcript of professional pharmacy education
• Three letters of recommendations from members of the pharmacy profession
• Personal interview
All application materials must be received no later than January 15, 2010. Our timeline for
processing applications, interviewing, and meeting ASHP Match deadlines follows:
• January 15, 2010: Application materials deadline
• January 31, 2010: Application review completed, applicants notified
• February 2010: On-site interviews conducted
• March 5, 2010: National Matching Service submission deadline
• March 17, 2010: Match results received, applicants and sites notified
