Baylor University Medical Center Dietetic Internship Program

Selection Criteria

The selection criteria for candidates for the Dietetic Internship are:

·Bachelor's degree by start date of the internship program.

·Commission on Accreditation for Dietetic Education’s minimum academic requirements completed by start date of the internship program.

·Overall grade point average of 3.0 or better on a 4.0 scale.

·Dietetic/nutrition-related course work grade point average 3.0 or better on a 4.0 scale.

·Activities that demonstrate leadership involvement and the ability to work well with people.

·Foodservice and/or hospital work experience is essential. Other work experience is helpful.

·Reference letters from professors and employers. (Major professor in food and nutrition and one in food service management, as well as any current employer.)

·Applicant's personal letter

·Final acceptance is contingent upon successful completion of the Baylor employee health physical and drug screen and background check as per procedure for all people working at Baylor University Medical Center.

·Health insurance is required of all students. Students must acquire insurance on their own.

·Proof of automobile liability insurance for travel is required of all students.

·The malpractice insurance provided by Baylor University Medical Center for employees covers dietetic interns.



How to Apply
 

Each intern application must include:

  1.  Dietetic Internship Program Application and Reference Forms
  2. Complete forms and include in packet.
  3. Letter of application typed or neatly printed in ink:
    1. Preferably not more than two (2) pages in length on plain white paper.
    2. Use business letter format.
    3. Include statement(s) of career goals.
  4. Official transcript(s) of credit from ALL universities and colleges attended.
  5. Signed and completed Declaration of Intent to Complete Academic Requirements OR signed and completed Verification Statement of Completion of Academic Requirements if applicant has already received a degree (provided by college).
  6. Signed agreement that information contained in the application is true and correct.
  7. Signed references from three to five professional persons qualified to give pertinent information as to the ability, potential professional qualifications, character and personality of the applicant. The references must include a major professor in food and nutrition and one in food service management. Work references are extremely desirable in addition to the major professor letters. Please duplicate the attached statement and give to individuals providing references. ORIGINAL SIGNATURES ARE REQUIRED ON ALL REFERENCE FORMS.
  8. A non-refundable application-processing fee of $75 must accompany this application. Please send a check or money order (not cash) made payable to Baylor University Medical Center.
  9. To ensure receipt of the application packet, send by Registered or Certified Mail. Applications must be postmarked NO LATER THAN FEBRUARY 16, 2010. Applications postmarked after this date will NOT be considered. Please allow adequate time for mail delivery.
Please make every effort to have ALL of the required materials listed on the application form in your packet when it is mailed. If any items are being mailed separately, please list and explain why. Recommendations mailed separately must have instructor's signature on back of envelope. If possible, include recommendations in packet. It is preferred that all forms be typed or neatly printed in ink.

·Applications should be sent Registered or Certified Mail to ensure delivery and acknowledgment of receipt. A self-addressed, stamped postcard may be included if notification of receipt of the application is desired. No other acknowledgment of receipt of the application packet will be made.

·Applications become the property of Baylor University Medical Center and will not be returned.

Verification of Completion of Academic Requirements
 

·All applicants must furnish a Verification Statement if a degree has been granted or a Declaration of Intent to Complete Academic Requirements if a degree is yet to be completed. Both forms may be obtained from your didactic program director.

·All applicants accepted into the internship must furnish a Verification Statement and Final Transcript upon completion of academic requirements and receipt of degree within 30 days of the start of the program.
