Requirements for Application:
- Doctor of Pharmacy Degree or equivalent
- Eligibility for Texas Licensure
o Must be registered as a Texas Pharmacist Intern prior to beginning program
o Apply now, there is no fee associated with registering as a Pharmacist Intern

o Nicotine-Free Hiring Policy – Matched Candidates that test positive for nicotine will have the offer rescinded
o Successful Background Check

Baylor University Medical Center participates in the Pharmacy Online Residency Centralized Application Service (PHORCAS) with an application deadline of Jan. 3, 2018.

- A letter of intent
o Please detail your reasons for your interest in completing a pharmacy practice residency, your short- and long-term career goals and why you believe our program will assist you in meeting those goals
o At least one of these must be from an employment supervisor
- Academic transcripts for professional pharmacy education
