Tammy S. Cohen, BS, PharmD, MS, FASHP, FTSHP, FACHE is the Vice President of Pharmacy Services at Baylor Scott and White Health (BSWH) in Dallas, Texas. Her responsibilities span pharmacy practice across many settings to include ambulatory care, inpatient and retail practice sites. She oversees the pharmacy services of the 22 hospitals and 28 retail pharmacy locations within BSWH. Tammy leads and serves on many multi-disciplinary committees and has been instrumental in leading financial and operational efficiencies that cross the organization and implementing clinical services.
Tammy has been active in several pharmacy organizations. She has been a member of the Texas Society of Health-System Pharmacists (TSHP) Board of Directors in various capacities since 2006; most recently she is serving as the President-Elect. She is also currently a member of the American Society of Health- System Pharmacists (ASHP) Section of Multi-Hospital Executives Committee. Additional previous TSHP roles include: serving as chair of the Council of Organizational Affairs, Chair of the Section of Pharmacy Management, member of the TSHP Education Council and as the TSHP representative to the Texas State Board Pharmacy (TSBP) Task Force on Internships.
Additional organization activities include formal roles with ASHP and the local societies of health-system pharmacy along with numerous regulatory related task forces. ASHP experiences include serving on the Council on Legal and Public Affairs, a member of the steering committee for the ASHP Leadership Meeting, the Chair of the Advisory Group on Publications for the Section for Inpatient Practitioners and a member of the Section of Financial Management for the Section of Pharmacy Practice Management.
Sidney Phillips, PharmD, MBA

Sidney P. Phillips Pharm.D, RPh, MBA is currently the Director of Pharmacy for Baylor University Medical Center in Dallas and the Division Director of Hospital Pharmacy for Baylor Scott and White Health-North Texas Division. Prior to moving into his current leadership role, he worked for Cardinal Health as Regional Director of Pharmacy Practice and Operations Director for Cardinal Health Pharmacy Solutions. Sidney has worked with over 20 different hospital systems and over 100 individual hospitals in Texas and throughout the United States.
Sidney grew up on a cotton farm and cattle ranch in Jayton, Texas. He received his Doctor of Pharmacy from Texas Tech School of Pharmacy. His pharmacy training was completed through an ASHP accredited residency in pharmacy practice at St. Luke’s Episcopal Hospital in Houston, Texas and a pharmacy management specialty residency with Cardinal Health.
Sidney is currently a member in the metroplex chapter of TSHP and has been a previous member in the panhandle, gulf-coast, and Lubbock area chapters of TSHP. He currently serves as the President of TSHP.
He has previously served as the section chair and vice-chair for TSHP’s Professional Affairs council, vice-chair for Public Affairs, and Secretary for TSHP’s Management section. Sidney is a past president of the board of directors for Texas Tech School of Pharmacy Alumni Association. Sidney has received TSHP’s Lourdes M. Cuellar Award for his service and Texas Tech School of Pharmacy Distinguished Alumni Award.
Rema Thyagarajan, PharmD, BCPS
Director of Pharmacy – Clinical Services
Rema Thyagarajan, Pharm D, BCPS, serves as the System Clinical Director of Pharmacy for Baylor Scott & White Health. She is responsible for the development and implementation of clinical initiatives that are patient-centered and improve business outcomes. In her role, she also develops data driven metrics to identify gaps and implement interventions to improve results on key clinical performance measures. She oversees and participates in the system pharmacy and therapeutics council and the system antimicrobial stewardship committee as well.
Dr. Thyagarajan is an experienced pharmacist by training and she has held several different positions throughout her career. She previously was the Director of Pharmacy at the Baylor Heart & Vascular Hospital and at one point before that she served as a Clinical Coordinator for internal medicine and surgery at Baylor University Medical Center. She is also a member of the American College of Clinical Pharmacy, American Society of Health-System Pharmacists, and the American Pharmacists Association.
Dr. Thyagarajan is a graduate of The University of Illinois where she earned a Bachelor of Science degree in biochemistry. She then went on to complete her Doctor of Pharmacy from The University of Texas at Austin. She also is a board certified pharmacotherapy specialist (BCPS).
Jason Trahan, PharmD
Pharmacy Director – Medication Safety
Residency Director - PGY2 Health-System Pharmacy Administration
Jason Trahan, PharmD serves as the Director of Pharmacy in Medication Safety for the Baylor Scott & White Health system and well as the Residency Director for the PGY2 Health-System Pharmacy Administration residency program. He is responsible for leading pharmacy medication safety initiatives in multidisciplinary teams, including the Patient Safety Committee and System Pharmacy and Therapeutics Committee. His role includes analyzing medication variance and adverse reaction data, optimizing medication libraries for IV pumps, as well as leading the Pharmacy Medication Safety Team and Residency Directors meetings.
Dr. Trahan has served in previous leadership positions including Manager of Medication Safety for the Baylor Healthcare System and Manager of Clinical Pharmacy Services at Baylor Scott & White All Saints Medical Center in Fort Worth, TX. During his time at Baylor All Saints, he also served as the PGY1 Residency Program Director. He has received recognition for his service and leadership including the 2009 Texas Society of Health-System Pharmacists Pharmacy Mentor Award and the 2010 CEO Service Excellence Award for Baylor Health Care System.
Jason Trahan received his Doctor of Pharmacy degree from Texas Southern University in Houston, TX and went on to complete a Pharmacy Practice Residency at Methodist Medical Center in Dallas, TX. He is a member of the American Society of Health-System Pharmacists and the Texas Society of Health-System Pharmacists.
David Watson, RPh
Director of Pharmacy Informatics
David Watson, BS, RPh serves as the Director of Pharmacy Informatics for the Baylor Scott & White Health system. He is responsible for leading the team of Informatics Pharmacists (IRX) in improving the use of medication management technologies through system design, workflow optimization, data analysis and pharmacy practice advancement. His roles include leading the EHR Pharmacy User Group (EPUG), Med Management Subgroup of Workgroup 1 (Governing body for BSWH Allscripts), and weekly meetings with EHR builders and the IRX Team. Additional responsibilities include participation with the Clinical Decision Support Subgroup.
Mr. Watson has served in previous leadership positions including Director of Pharmacy at Baylor Medical Center at Grapevine and Vice President of Technology with Pharmacy Healthcare Solutions, a subsidiary of AmeriSource Bergen.
David Watson received his Bachelor of Pharmacy degree from Southwestern Oklahoma State University in Weatherford, OK. He is a member of the American Society of Health-System Pharmacists and the Texas Society of Health-System Pharmacists.
Heath Huston, MBA
Pharmacy Project Analyst
Heath Huston, MBA serves as the Pharmacy Project Analyst for Baylor Scott & White Health system in the department of Pharmacy Administration. He is responsible for managing the entire drug spend portfolio to find utilization and contracting savings and has worked with the metrics committee to create and maintain a metrics database of key performance indicators for all pharmacies throughout the system.
Mr. Huston has previously served as a Financial Analyst at Baylor Healthcare System where he participated in pharmacy lean savings initiatives, created and maintained a database used to prepare Cardinal distribution and savings reports for all facilities, and prepared monthly supply variance reports for supply chain operations directors.
Heath Huston received his Bachelor of Business Administration at Abilene Christian University and went on to earn his Master of Business Administration at Texas Christian University.
Leon Jacobs, CPhT
Pharmacy Program Manager – Procurement
Leon Jacobs, CPhT is the Pharmacy Program Manager of Procurement for Baylor Scott & White Health system. His responsibilities include coordinating drug purchases and related support across the system by working with facility buyers and system management leaders, maintaining contract compliance, and identifying potential cost savings opportunities. He interacts with pharmaceutical company representatives and assists with implementing new contracts, processes, and vendors. He also manages all drug recalls and drug shortages throughout the system. Leon was recognized by the Texas Society of Health-System Pharmacists in 2016 for his exemplary accomplishments in fostering the advancement of patient care and the profession of pharmacy and was presented with the Mike Knapp Pharmacy Technician Award.
Mr. Jacobs has previously served as a Pharmacy Technician as well as the Pharmacy Purchasing Representative at Baylor University Medical Center in Dallas, TX where he coordinated the purchasing of medications and supplies and assured that the pharmacy management team was informed of drug shortages, withdrawals, and potential cost saving alternatives. He also has experience as a Formulation Development Supervisor for BASF (formerly Knoll Pharmaceuticals) in Shreveport, LA.
Leon Jacobs is a Board Certified and Texas registered pharmacy technician. He is a member of the Texas Society of Health-System Pharmacists (TSHP), the National Pharmacy Purchasing Association, and is a TSHP Technician Executive Committee Member.
